Position Summary:

The Resident Support Lead (RSL) works within a comprehensive, multidisciplinary, person-centred model of care that integrates social, recreational, relational, and medical support. RSLs provide leadership, mentorship, and support to the long-term care home team, helping to develop and carry out comprehensive plan of care. The RSL contributes observations and collaborates with the care team to model, coach, and mentor the implementation of agreed-upon strategies and other relevant interventions for managing responsive behaviours. Through a strong commitment to resident-centred care, this role helps foster a positive, non-judgmental, creative, respectful, flexible, and enthusiastic culture.

The Resident Support Lead is a member of the embedded Behaviour Support Ontario (BSO) team for Wiigwas and will also work collaboratively with the Thunder Bay BSO Team and CMHA Fort Frances Team.

Overview of Responsibilities

  1. Clinical Leadership and Resident Support
  • In collaboration with Nursing Administration, oversees the implementation of therapeutic interventions and supports the implementation with the Personal Support Workers caring for residents with responsive behaviours.
  • Use the BSO 5 STaR model to promote team-based, collaborative, evidence-informed, and holistic care that supports positive culture change and improved outcomes.
  • Provides recommendations on bio-psycho-social and non-pharmacological interventions that support resident goals related to psychosocial needs.
  1. Assessment, Referrals, and Care Planning
  • Completes required assessments and develop strategies and recommendations for identified residents, including pre-admission, admission, transitional, and episodic cases, from case opening to completion within the assigned long-term care home.
  • In collaboration with the care team, identifies the need for additional consultation and prepares referrals to other services or specialists, as required, such as a Psychogeriatric Resource Consultation.
  1. Quality Improvement and Reporting
  • Maintains service indicator data, statistics, and reports to support quality improvement initiatives and organizational reporting requirements for the Behavioural Supports Ontario program.
  • Participate in continuous quality improvement activities identified by the Director of Resident Care, Wiigwas Leadership, or the Leadership of the Regional BSO Lead, including data collection, analysis, improvement initiatives, and PDSA cycles.
  1. Collaboration and Professional Participation
  • Participates in responsive behaviour meetings and attends staff meetings, education sessions, and required training.
  • Performs other duties as assigned.

Qualifications, Skills and Experience

  • Diploma or degree in one of the following – Social Services, Psychology, Therapeutic Recreation, Personal Support Work, Developmental Support Work, Practical Nursing, or related fields.
  • Experience in Long-term Care or working with older adults is an asset.
  • Demonstrated knowledge of and ability to apply best practices in supporting people living with dementia.
  • Experience in mental health or related practice areas is preferred.
  • Knowledge of or experience working with Residential School Survivors is an asset.
  • Experience with electronic documentation, including the Resident Assessment Instrument and care planning functions, is required.
  • Training in the Behavioural Supports Ontario 5 STaR approach to responsive behaviours is preferred. Candidates must be willing to complete all required training, including BSO Foundations, PIECES™, U-First!®, Gentle Persuasive Approaches™, and Dementiability™.

Conditions of Employment

The successful candidate will be required to:

  • Provide a satisfactory Vulnerable Sector Check (VSC) prior to commencing employment.
  • Provide satisfactory 2step Tuberculosis (TB) screening/testing in accordance with the Fixing Long-Term Care Act, 2021 and organizational policy.
  • Successfully complete all mandatory orientation and Surge Learning requirements within the timelines established by Wiigwas Elder and Senior Care.
  • Meet all other pre-employment and ongoing employment requirements as determined by Wiigwas Elder and Senior Care.
